Laser or surgery? This question comes up at every consultation with a patient seeking treatment for dark circles. The confusion is understandable: online, you can find everything and its opposite. Some colleagues promise “dark circles erased in one laser session,” while others systematically recommend surgery. The clinical truth is more nuanced. After more than 30 years dedicated to eyelid and peri-orbital region surgery, I can state it clearly: there is not ONE type of dark circle—there are three. And each type requires a different treatment. Here is what you need to understand before choosing.

The three types of dark circles: a diagnosis first

Before even discussing treatment, a diagnosis must be established. This is the most important point, and paradoxically the one that many patients do not receive at their first consultation elsewhere. There are three main categories of dark circles, which can coexist in the same patient.

A fourth presentation, common, is mixed dark circles, where two mechanisms coexist (for example hollowing + pigmentation). Laser: a specific indication, not a magic wand

The Q-switched (switched) laser is currently the reference technique for treating pigmented dark circles at early to moderate stages. Its principle: emit ultra-brief pulses that shatter melanosomes subcutaneously without damaging surrounding tissues. The melanin fragments are then eliminated by the body’s macrophages in the weeks following.

Specific indications for Q-switched laser

  • Pigmented dark circles at early stages (I and II on a IV-point scale)
  • Hereditary dark circles with ethnic component (common in Mediterranean, Asian, or South Asian patients)
  • Post-inflammatory dark circles (sequelae of eczema, contact dermatitis)
  • No associated structural hollowing

What laser does NOT treat

  • Hollow dark circles (no volume to add)
  • Pure vascular dark circles (Q-switched laser targets melanin, not hemoglobin)
  • Eyelids with marked skin excess (surgical indication)
  • Malar bags and subpalpebral fat bags

Laser protocol

3 to 5 sessions spaced 4 to 6 weeks apart. Each session lasts 10 to 15 minutes, under EMLA cream anesthesia. Recovery is discreet (mild redness for 24 to 48 hours). Results are progressive, visible from the 2nd session and optimal 2 to 3 months after the last one. Strict sun protection is mandatory throughout treatment.

Surgery: when and why

Surgery of the peri-orbital region is not strictly speaking a treatment for dark circles: it addresses the anatomical causes that produce the dark circle effect. When a patient presents with deep hollowing, subpalpebral fat bags, or skin excess, no laser in the world will correct the problem. Surgical intervention is needed.

  • Skin excess with deep wrinkles and crepey skin (indication for lower blepharoplasty)

Surgical techniques for the eye area

  • Dark circle lipofilling: restoration of lost volume, without incision, under local anesthesia. Ideal indication for isolated hollow dark circles.
  • Transconjunctival lower blepharoplasty: removal of fat bags via internal approach (invisible scar). Indication: fat bags without skin excess.
  • Skin-level lower blepharoplasty: skin removal + SOOF repositioning. Indication: marked skin excess.
  • Combined technique (pinch blepharoplasty): transconjunctival approach + skin pinch. Indication: combination of fat bags + moderate skin excess.

Clinical cases from the Hayot practice

Case 1 — Ms. S., 32 years old, ethnic dark circles. Patient of North African descent with marked pigmented dark circles, without hollowing, without skin excess. The mistake would be to operate. I proposed a protocol of 4 Q-switched laser sessions spaced 5 weeks apart, combined with a kojic acid lightening cream for maintenance. Excellent results at 3 months, maintained at 2 years with one annual maintenance session.

Case 2 — Mr. J., 46 years old, structural hollow dark circles. This patient had undergone three laser sessions with another physician with no results. Normal: he had no pigmentation to treat. Examination showed deep hollowing with tear trough and early fat bags. Clear surgical indication. I performed dark circle lipofilling combined with pinch lower blepharoplasty. Stable result at 18 months, delighted patient.

Case 3 — Ms. V., 38 years old, mixed dark circles (hollow + pigmented + vascular). Common situation. Sequential treatment plan: first lipofilling for dark circles to correct the hollowing and thicken the skin (trophic effect of associated nanolipofilling), then 3 Q-switched laser sessions at 3 months to treat residual pigmentation. Chronology is important: always treat the hollow before pigmentation because skin thickening after lipofilling spontaneously reduces the vascular component.

Decision tree: laser or surgery for your dark circles?

Profile 1 — Dark brown dark circles without hollowing (20-35 years old)

Q-switched laser. Pigmentation is the ideal target. No surgery needed. Cosmetic support with strict sun protection is essential.

Profile 2 — Visible hollow dark circles, worsened by fatigue (30-50 years old)

Surgery (dark circle lipofilling). Laser is of no use here. The surgical procedure is minor (local anesthesia, 45 minutes, social downtime of 5-7 days) and provides durable results.

Profile 3 — Marked fat bags (40-60 years old)

Surgery (lower blepharoplasty). No laser treats fat bags. A consultation is essential to choose between transconjunctival or skin approach.

Profile 4 — Pure purplish/bluish dark circles (all ages)

Nanolipofilling + PRP. Q-switched laser is not indicated here (it targets melanin). Nanolipofilling thickens the skin and masks vascular transparency.

Profile 5 — Complex mixed dark circles

Combined and sequential treatment plan. Always treat volume before pigmentation. Expect 6 to 9 months for the complete protocol.

Consultation (45 minutes)

Diagnosis is the heart of the consultation. I examine the lower eyelid at rest and during orbicularis muscle contraction, evaluate skin transparency, and look for a vascular component by pressing on the skin. A standardized photographic assessment is performed. At the end of the consultation, the treatment plan is established and a detailed quote is provided.

The procedure

Q-switched laser: in the treatment room, under anesthetic cream. 10 to 15 minutes per session. Surgery: in the operating room, under pure local anesthesia or with sedation. Duration 45 minutes to 1.5 hours depending on extent. Discharged same day.

Post-operative recovery

Laser: redness for 24 to 48 hours, makeup the next day. Surgery: Days 0-3 edema and bruising, Day 7 return to social activities with makeup, Day 15 follow-up consultation, Day 30 nearly final result, Day 90 stabilized result.

FAQ — Your frequently asked questions

Can laser replace dark circle surgery?

No, never, if the indication is hollow dark circles or fat bags. Laser treats pigmentation, nothing else. Promising otherwise is an indication error.

What is the price of a Q-switched laser session?
