Pockets are the result of a decrease in collagen in the skin, often linked to age. In the orbit, the eye is surrounded by a fatty cushion that protects it from the bone walls, and is held in place by a very thin tissue called the septum, which tends to distend with age. The fat then slides downwards, forming pockets, or fat hernias.
