A medical assessment

Analysing
before acting

Expression lines, loss of volume, skin quality and laxity do not call for the same approaches.

The site clearly separates the families of procedures in order to make the information easier to follow. The products, technologies and modalities proposed are confirmed by the doctor.

What the consultation assesses

The consultation clarifies the request, the areas concerned and the medical history. It also examines the skin, the volumes, the mobility of the face and the balance between the eyelids, the eyebrow and the neighbouring tissues.

Dr Bernard Hayot practises as an ophthalmologist, with a practice oriented towards oculoplastic surgery. When a request concerns an area outside the periorbital region, the procedure is carried out four-handed with a surgeon qualified for such procedures.

The examination makes it possible to distinguish a reasonable medical indication from a request that calls for another solution or for abstention. The personalised schedule, the precautions and the follow-up are determined during the consultation.

The families
of approaches

Dosages and modalities are never standardised on the site: they are determined individually during the consultation. Results vary according to the anatomy, the indication and the response of the tissues.

01

Fat micro-injections

Autologous fat may be discussed in order to restore a volume or to support the quality of the tissues. The harvesting, the preparation and the recipient area answer to a precise indication.

For an area outside the periorbital region, the procedure is carried out four-handed with a surgeon qualified for such procedures. Results vary and an alternative may be preferable.

02

Botulinum toxin

Botulinum toxin acts on certain muscles responsible for expression lines. The analysis covers the dynamics of the eye area, the position of the eyebrow and the asymmetries at rest as well as in movement.

The indication, the dosage and the injection points are determined during the consultation. A request outside the periorbital region is carried out four-handed with a surgeon qualified for such procedures.

03

Hyaluronic acid

A filler may be considered in order to support a hollow or to correct a transition of volume. The tear trough, the lips, the nose and the folds present distinct anatomies and risks.

Outside the periorbital region, the procedure is carried out four-handed with a surgeon qualified for such procedures. The choice of product remains subject to medical validation and results are variable.

Hyaluronic acid The lips

04

Correction & hyaluronidase

A previous injection may call for simple monitoring, a targeted correction or another strategy. The examination seeks to establish the nature of the product, the area treated and the signs that would require appropriate management.

The possible use of an enzyme is neither automatic nor trivial. Outside the periorbital region, it is carried out four-handed with a surgeon qualified for such procedures.

Hyaluronidase

05

Lasers & technologies

Technologies may target texture, certain pigmentary irregularities or laxity, according to the equipment actually available. The skin type, recent sun exposure and medical history guide the discussion.

No device is presumed suitable before the examination. Outside the periorbital region, the treatment is carried out four-handed with a surgeon qualified for such procedures.

06

Other treatments

Other approaches to the skin or to the contours may be raised only after confirmation of their indication and of the care setting. Their value is compared with monitoring, with medical care or with a surgical option.

Any procedure outside the periorbital region is carried out four-handed with a surgeon qualified for such procedures. The personalised schedule is determined during the consultation.

Frequently asked questions

How does one choose between injection and surgery?

The choice depends on the anatomy, on the discomfort expressed and on what a procedure can reasonably change. The consultation also presents abstention and the alternatives, with variable results.

Is a photograph enough to propose a treatment?

No. A photograph may document a request, but it does not replace the examination of the tissues, of the movements, of the medical history and of the contraindications.

Are the treatments suitable for every skin?

No. The skin type, its sensitivity, the medical history and recent sun exposure may change the indication or lead to postponing the procedure.

How are the products and devices chosen?

They are confirmed according to the indication, their availability and the relevant medical data. No product or device is considered appropriate before this validation.

Can several areas be treated within the same course of care?

Coordinated management may be discussed, without any automatic addition of procedures. The personalised schedule is determined during the consultation according to the priorities and the necessary precautions.

What should be reported before an aesthetic medicine procedure?

Medical history, allergies, current treatments and previous injections or procedures must be disclosed. Any unusual reaction or recent change in the state of health must also be mentioned.

What happens if the indication is not accepted?

The doctor explains the limits observed and may propose monitoring, an alternative or the absence of treatment. A request outside the periorbital region falls under four-handed management with a surgeon qualified for such procedures.

Are the results predictable?

Results remain variable and cannot be guaranteed. The prior information distinguishes the objective discussed, the limits of the procedure and the possible after-effects.

A consultation,
not an algorithm

The site helps to prepare questions, but proposes no treatment “suited” to you on the basis of age, sex or a photograph. The indication depends on a medical examination and on personalised information.

Limits and alternatives

A non-surgical treatment does not correct every cause of a dark circle, of an under-eye bag, of a line or of laxity. Monitoring, medical care, surgery or the absence of any procedure may be more consistent with the situation.

The risks, contraindications and warning signs are explained before any decision. Outside the periorbital region, the procedure is carried out four-handed with a surgeon qualified for such procedures.

Benchmarks from the literature

What the studies say

A few figures published in peer-reviewed journals, chosen to give an idea of the orders of magnitude. They do not describe your case: only the consultation does that.

  1. For glabellar lines, onabotulinumtoxinA, a botulinum toxin type A, acts rapidly, the maximum effect being observed between the thirtieth and the sixtieth day. In a narrative review of the registration trials, the median duration of the response on dynamic lines was 120 days; these durations cannot be generalised to all type A toxins.

Carruthers et al., Medicine (Baltimore), 2023 · PMID 37499082
  2. Two randomised, placebo-controlled phase 3 trials which included 609 participants and assessed a long-acting formulation of botulinum toxin type A (daxibotulinumtoxinA) on glabellar lines report that a severity of none or mild was maintained for a median duration of 24 weeks. The most frequent treatment-related adverse effects were headache and pain at the injection site.

Bertucci et al., Journal of the American Academy of Dermatology, 2020 · PMID 31791824
  3. Hyaluronic acid has the particularity of being reversible: a systematic review devoted to hyaluronic acid after blepharoplasty, based on five studies of 5 to 109 patients of heterogeneous methodology, reports that hyaluronidase makes it possible to treat persistent oedema with an improvement of the order of 50 to 100 % according to the studies, which distinguishes this product from fat transfer, which is not reversible.

Safia et al., Journal of Clinical Medicine, 2025 · PMID 40648944
  4. Visual impairment after the injection of a filler is a serious and documented complication: the published cases number in the hundreds, the sites most at risk being the nose, the forehead and the glabella. A 2020 review analysed 50 of them in detail, which justifies a precise knowledge of the vascular anatomy of the periorbital region.

Lee et al., Journal of Cosmetic Dermatology, 2020 · PMID 31709739
  5. An updated review of visual impairment after the injection of a filler lists 365 new cases published between September 2018 and March 2023; the sites most at risk were the nose (40.6 %), the forehead (27.7 %) and the glabella (19.0 %), the product was a hyaluronic acid in 79.6 % of cases, and 68.2 % of the patients whose outcome is reported did not recover their vision.

Doyon et al., Aesthet Surg J 2024 · PMID 38630871
  6. A systematic review of 23 studies devoted to fillers of the tear trough reports a high level of satisfaction and a duration of effect of 8 to 12 months, with generally mild adverse effects: bruising, oedema, blue-grey discolouration and contour irregularities.

Trinh et al., Facial Plast Surg 2021 · PMID 34192769
  7. A systematic review of 42 studies comparing the injection of hyaluronic acid into the tear trough with a needle and with a cannula finds no significant difference in aesthetic outcome, in satisfaction or in the frequency of adverse effects, the majority of the works being of level III evidence.
