Data collected through the contact form

The contact form collects only the information you enter in it: surname, first name, email address, telephone number and the content of your message.

The form does not require any information about your state of health. Enter only what is necessary to book an appointment: this website is not an approved health data host within the meaning of Article L.1111-8 of the French Public Health Code.

Photographs

The consultation form does not allow photographs to be attached and does not require any information about your state of health. No medical opinion is given without a consultation: it is established during a consultation, at the practice or by teleconsultation.

Photographs useful for a teleconsultation are sent after the appointment has been booked, as directed by the secretariat. These are data concerning health: they are processed only with your explicit consent (Article 9.2.a of the GDPR), which you may withdraw at any time, and they are neither published, nor used for communication purposes, nor transmitted to any third party. The secretariat deletes them at your request.

Purpose and legal basis

Purpose: to respond to your request for an appointment or information.
Legal basis: your consent (Article 6.1.a of the GDPR), collected through the tick box on the form, together with the pre-contractual measures taken at your request (Article 6.1.b).

Recipients

Your data is intended solely for Dr Bernard Hayot and, where applicable, the practice's secretariat. It is not sold, rented or transmitted to third parties for commercial purposes. No transfer outside the European Union takes place.

Retention period

Messages received via the form are kept for the time necessary to process the request, then for 12 months from the last contact, before deletion.

The medical record compiled as part of care falls under a separate regime and is kept in accordance with Article R.1112-7 of the French Public Health Code.

Your rights

You have a right of access, rectification, erasure, restriction, objection and data portability. You may withdraw your consent at any time.

To exercise these rights, send your request by post to the practice's address or by email to info@drhayot.com. A response will be provided within one month.

You may also lodge a complaint with the Commission nationale de l'informatique et des libertés (CNIL), 3 place de Fontenoy, TSA 80715, 75334 Paris Cedex 07, cnil.fr.

Security

The site is served over HTTPS. The technical and organisational measures implemented are designed to protect your data against unauthorised access, alteration or disclosure.
